What are most members opinion on the level of sterilization when it comes to gynecological procedures?
The obvious general guidelines come to mind
1) Use latex gloves
2) Use sterilized equipment - does the overall genital area need to be disinfected prior to any medical play? Alcohol wipe, etc?
3) What about other guidelines?

NB: In the past I have stated that everything should be sterile, but this question goes into specifically what is needed in certain instances, so my responses may be different from what I have said before. All things being equal, sterile would be best, but not always necessary.
The level of sterilization for gynecological procedures varies greatly depending on what you are doing.
If you are using the instruments on only one person, and you are fluid bonded, you do not need sterilized instruments when inserting anything into the vagina. You do not need latex gloves either, but your hands, like your penis should be washed prior to entering. (You don't use condoms or latex gloves when you finger your SO, do you?) The vulva should be washed well with soap and water.
Insertion of instruments into the urethra is another matter. If you are using them on only one person, those don't need to be sterile either, but should be what is considered "surgically clean."
Anything that has been in the rectum should be scrupulously cleaned (sterile is better) before insertion into the vagina.
Once the instruments are used in more than one person, they must be sterilized to prevent spread of infection. You don't want to transmit hepatitis or other serious infection, do you?
DO NOT INSERT ANYTHING THROUGH THE CERVIX REGARDLESS OF ITS STERILITY--YOU DON'T REALLY KNOW WHAT YOU ARE DOING. To paraphrase an old sage, Just because you think you know the tricks of the trade, that doesn't mean you know the trade.
