I've been asked to tie a patient up, beat them, cane them, fix painful clamps etc. but I will certainly not do this with a new patient nor will I do anything which is likely to endanger a patient on any part of their body. I might perhaps on request smack a bottom with my bare hand.

That’s exactly the reason why safe words exist, since when you’re in a play situation you can get so much into character that it can get really intense. There’s really no danger of hurting your patient, BIGK, if you use safe words, if at any point he/she feels things are too intense then all that’s needed to be done is to shout “amber” or “red” or whatever safe words you prefer. Hence, you are able to avoid hurting or endangering your client.
Besides, it’s not only by incorporating BDSM that the possibility of hurting or endangering your patient exists. Any procedure done without proper precautions can result in danger, accident or injury. And this isn’t only for non-medical professionals. Even healthcare professionals who play aren’t 100% foolproof. I mean, any unknown allergic reaction to any drug no matter how safe you play can result in injury, it’s true that being a healthcare professional makes it safer since you’d know what to do but that doesn’t mean you can’t make mistakes.

For a patient I have seen before and after properly counselling them I might agree to doing things which are to some extent painful BUT it has to be safe and sane AND have the potential to result in pleasure.

There’s a fine line between pain and pleasure, in fact the line is so fine it crosses over and even merges, that’s why BDSM and MEDFET work hand in hand. Also, it isn’t all physical, patients don’t ask to be tied down or spanked or beat just for the sake of feeling pain. A lot of what’s it about is psychology. BDSM is not just about the pain, it’s a lot about power, control and trust. Surrendering yourself to someone else’s control, trusting completely that you will both get pleasure from whatever scene it is. Medfet is a lot like that, even RL medical situations are in a way like that, submitting yourself to an exam, surrendering yourself to your doctor’s control, trusting that he/she has your best interests in mind. Also, half of why most are into medfet is because of the feelings of vulnerability or embarrassment that lets them get off, which is a lot like what subs in BDSM feel.
MEDFET and BDSM are very much alike. It isn’t just about pain.
As with my answer to the original question:

What is the most painful part for you females in the exam?
and for males?
though its painful do you enjoy it?
