This is my first time posting here.
I have been to Pelvic Floor Physical Therapy.
My OB/GYN suggested it as a possible solution for painful intercourse. When he brought it up I had no idea what it was. He said "there is something called pelvic floor physical therapy that I think may help - would you be willing to give it a try?" I said "sure, why not?" He said "well sometimes it involves internal work". I had no idea what he was talking about. I envision PT as a big room with lots of people stretching and moving. He prescribed 8 weeks of this therapy.
That's not what this is.
There is very little online about it. I didn't have a clue what would happen. There are very few place that actually even do it. I read on the website to the clinic I was going to that it's a warm environment "no exam tables or stirrups".
I walked in to the building it and it was very warm - no typical doctor's office furniture, they had a puzzle out to do while you were waiting, the receptionist introduced herself and offered me tea.
Then the physical therapist came out lead me back into a room. It was private room. A very home-like environment. Again no typical office furniture. The room was decorated like a living room. It was dimly lit with table lamps. The room did have a very sterile white sink and a long cushioned table (the kind table you'd imagine for physical therapy) She asked me loads of questions. Really personal questions. Then she said "we'll be doing mostly internal work, I'd like to start with an evaluation and see where we are. Do you feel comfortable starting today?" I thought geez why not I'm already here. So she had me stand up and do some movements. And had me lay on the table and she pressed against my pelvis and did some more movements. I thought to myself. What's the big deal about this?
Then it came.
She pulled a bedsheet from the drawer. It had flowers on it. She said "it's time to see whats going on inside". She had me get down off the table and covered the table with the bed sheet. She said "I need you to remove all your clothing from the waist down and lie down on the table . I will be back in a few minutes. She handed me a stark white sheet and said to use it to cover up.
I did I was told. She came back and it and explained that today she would be doing an evaluation and then in the upcoming appointments we would begin treatment. She put on a glove and asked me to raise my my legs slightly and open my knees wide. She said she would start by just examing the outside anatomy and then move inside. And that she did. She lifted up sheet and do a thorough examination of my vagina. Then she put some lube on her fingers and inserted one and then added another. She started just going in a little bit and then eventually went as far as her fingers would allow. At various times she would press or move her fingers and ask if there was any pain. The it was much more thorough than any exam I've gotten at any doctor's office. Twenty minutes went by and her phone started making an alarm. She removed her fingers and announced that our time was up. She said " I feel that there is a lot of work to be done". We will spend the majority of next work working those muscles. And the following week we will do some biofeedback. I had no idea what she was talking about. She took some wipes and said I could get dressed. She said I could use the wipes to clean up if I needed.
The next week when I came in she asked a few brief questions and she had me do some movements and then said we would spend the majority of our time doing internal work. Again she asked me to undress from the waist down and lie down on the "bed" as she called it. When she came in she put in her glove and said she would start working on stretching those muscles. She put her fingers right in and pressed around. She said she was stretching the muscles out. She said she could feel the tightness and understood why intercourse would be painful. For a good 3O minutes she 'internal work' continued. She made small talk during the whole thing. When the session was over she told me that next time we would work on biofeedback.
The next week came. She said since we were doing the biofeedback we would get started right away. This time, on the cart near the table, there was a computer in the room with a bunch of wires connected to it. It looked similar to a portable EKG machine. I again undressed and layed down on the table. She came in shortly and explained we would be using the biofeedback machine to identify where the pain was and the strength of the pain. I would be lying if I said I wasn't a little bit terrified.
She explained that she would be placing some pass on the wires and the wire would be placed on my butt-cheeks near my anus and then she would do the internal work at the same time. She lost me as soon and she said anus so I really have no idea what the machine was supposed to do or why.
She told me to turn over on my left side. She opened a drawer and connected some pads to the wires. She said before she proceeded she would need to do a brief examination to make sure there were no sores or lesions in the area. She lifted up the sheet so that my entire bum was exposed and asked me to reach around a lift up my "cheek". She ran her gloved finger around my anus and inserted it into briefly inside. She then changed her gloves and placed on the pads on each of my cheeks. I could feel the edges of the pads right up against my rectum. She then had me roll back over. I could feel the wires between my legs. She lowered the sheet slightly exposing the top of of my pelvis and placed two additional pads. She then had be contract my muscles several times (similar to doing Kegal exercises) and then continued with similar "internal work" as she had done in the prior visit. This time she carefully studied the screen during the entire visit.
Every week for the next 5 weeks I returned. Every other visit the pads would be attached. I knew the doctor had written the order for 8 weeks. On my 8th visit I asked the therapist if this would be my last visit. Little did I know she had already been in communication with my doctor to get the order extends. She said we had made some progress but felt we needed atleast another 8 weeks and the doctor had extended the order.
I went for an additional 2 weeks after that but then I stopped. The truth is the experience was incredibly uncomfortable (not in the physical sense) just kind of felt weird. The therapist often made an effort to make friendly conversation while she was doing the work - and that just made really uncomfortable. I understand that she did that in an effort to make things less uncomfortable, but for me it just made things weird.
So that's my story.
