Glad you found someone to do medical play with! At least in my experience, found most of my play partners here or fetlife. We had conversations about any previous fetish experiences and interests, what we desired in the play exam, along with things that were soft or hard limits. Definitely important to outline both things you strong desire in the play along with your limits. In some cases I met with them casually prior to doing any play (i.e. coffee, etc.), other times first time meeting we engaged in medical play. Just depends on what you are comfortable with.
Everyone's experiences will be different because we all have different preferences, equipment limitations, etc. I tend to prefer the patient role. As a patient, after spending a little bit just chatting with my play doctor, they have me get naked and then go through the exam. Most of my play docs have been good. They go through all the main examination things you would expect. I am huge into stethoscopes, so they will spend more time listening to my heartbeat as previously discussed. I also prefer lots of palpation, so having a good musculoskeletal and neuro exam are, from a sensory standpoint, very appealing to me too.
For me, medical play is sexual in nature; that said, it doesn't have to be specifically sexual for you or involve sexual contact if you don't desire such. I have always had a rectal exam performed (digital and anoscope), along with having the play doc examine my penis and testes. The play doc will also ask questions about my sexual health. They will then make me get erect, re-examine my genitals, and then stimulate me to orgasm 2 or 3 times. This is, again, my preference that was already discussed and consented to with my play doc.
Your exam is yours to create. Also, if you find something doesn't feel good to you, say something. Most people want their partner to have a positive experience and aren't hurt if you say no to something.
