Not sure this is quite what the OP had in mind, but in the realm of age play and … well, not mandatory exams so much as a lack of agency over what’s going to happen to her.
A general scenario I’ve been considering for writing purposes…
An older man, called daddy, arrives with a young woman, mid-20s, but behaving very submissively. They’ve come because of [insert complaint here]. The complaint could be abdominal pains, cramping, stress incontinence … whatever symptom that could lead to an intimate examination of whichever body system you choose to focus on.
Upon meeting the patient she almost completely defers to Daddy for all her answers. Now, you can make it legit if that’s important to you by having her in the room alone where you confirm her freedom/choice and she explains that she entrusts her care to her dominant partner. Or, as it’s fantasy … leave it out. Your call…
Do the basic exams, gradually working your way to the more private parts, but always provide a good running commentary of all your observations.
“A little bit of firmness here, she could be constipated- when did she last move her bowels?”
“Her breasts seem a little tender - is she menstruating right now?”
“Her genitalia look healthy, but there’s a bit of inflammation around her urethra, has she complained of any pain when urinating?”
“There’s nothing back here medically to worry about, but she could be a bit more thorough keeping herself clean. She’s got a bit of fecal matter there and it could cause some irritation if it happens too often. You may want to check her after she wipes, just to be sure she doesn’t miss anything.”
And - most importantly - talk almost exclusively to him. As though she had no agency. In fact, when it comes time to remove articles of clothing, direct him to do so.
“Can you have her remove her top for me please?”
“I’ll need her to lift her hips so I can lower her panties and take a look beteeen her legs.”
“She’s probably not going to like this very much, but I’d like to do a quick rectal exam.”
“I’d like to give her some antibiotics, just as a prophylactic measure. They’re a bit painful, so I’d recommend holding her over your lap while I administer them, you can help her hold still during the hard parts.”
Many possibilities, it’s all in how you choose to establish the permission structure.
