She has been rescued from nameless Bad Guys who forced her to swallow stolen gemstones in order to get them across the border undiscovered. Your job is to first do a thorough exam, which includes a rectal temperature, assessing her general health as well as whether she was sexually abused. Then you get to the more delicate part of your job, recovering the gems so they can be returned to their rightful owner, which requires at least one enema, and if the gems are not expelled, perhaps another, ending with a scope to see if they're soon to be delivered or perhaps stuck.
She is in a foreign country where she does not speak the language. (If you have even a few words of another language, now's the time.) She has been exposed to a serious illness and must be examined, her rectal temperature taken, her throat, ears, and vagina looked into, and without using any English, you tut-tut that she will need to have her colon cleaned before you can look for signs of the illness there. You pantomime what is about to happen as well as you can, but she may not comprehend and therefore doesn't cooperate, so you restrain her for an enema and again for the scope.
She is a commoner in a historical-fantasy setting, and your king has seen her and been quite smitten. Your job is to determine whether she is worthy of his physical attentions, which she does not have the right to refuse. You must disrobe her, wash her with a rag and a bowl of warm water (these peasant women are so filthy!), then look over every inch of her. She is embarrassed and frightened, and when you tell her you just use an invention from Someplace Far Away to determine whether she has a fever, she's even more scared once you grease her up for the thermometer. In a no-nonsense way, you announce she must be cleaned internally until you are satisfied, then you will use an amazing invention to look inside her--all for your king, of course.
