@Berko I have no direct experience with holes in diapers, but this is the sort of inventive puzzle I enjoy pondering. Do you want this diaper for the giver or the receiver to wear? Or maybe both?
Assuming the giver has an attached penis and not a strap-on, I would think a small hole properly placed would allow everything to stay put and work well. On the other hand, the receiver’s pucker is set back a bit between the cheeks, and body positioning as well as angle of approach will vary the ideal placement for the diaper hole. The hole may need to be larger since the diaper surface is farther away from the sphincter, and alignment will be rather unpredictable.
Cloth diapers sound good from a modification point of view, but maybe not as good from an absorption point of view. I would draw the circle you plan to cut out, then using a zig-zag stitch sew around the circle approximately a half inch outside the circle. That way when you cut it out there will be about a half inch of fabric available to adjust, fray, and soften. This will keep the opening gentle in contact. Zig-zag stitching is more flexible on stretchy fabric or in situations where you need the stitching to have give.
For disposable diapers with SAP I might try a glue approach. Fabric stores sell bias tape(not adhesive) which is a long strip of fabric folded a couple times. With a disposable diaper you might be able to cut the hole and then use fabric glue to attach the bias tape, concealing the cut edge. Glue one edge of the tape to the outside surface, curl around the cut edge and glue to the inside surface. Do not glue right at the cut edge to keep it soft as possible. Depending on the width of the bias tape this will help get the stiff glued areas away from the edge. This way the phallus will really only be in contact with the fabric bias tape at the fold, and not in touch with the hardened glue zone.
The bias tape will also provide quite a visual aesthetic, highlighting the hole and it’s intended purpose. This sounds like a fun craft project, leading to an even more fun activity. Keep us posted on your experiments. You could also put some photos into an album for us to see your handiwork.
