My wife and I don't have a joint annual physical exam, but we do have a joint annual skin exam.
Once a year, usually in late August, my wife and I visit our dermatologist to be checked for skin cancer. We schedule back-to-back appointments, but then we just both go in together and have one long appointment since I would attend my wife’s appointment and then she would attend mine.
The PA and her assistant, two women in their 30s, escort my wife and me into the exam room, and the PA hands each of us a folded-up paper gown, and instructs us to remove all of our clothing except our underpants for me and panties for my wife, unless I happen to be wearing a pair of my wife’s panties, in which case she would ask both of us to remove my wife’s panties, but I have never worn my wife’s panties, at least not to the dermatologist’s office. She tells us that when we are down to our skivvies, we are to don the gown with the opening in the front.
Before they leave the room so my wife and I can get nearly naked, I ask if we have time for a conjugal visit. For three years this request has been vetoed because at 67 years old, even a “quickie” for us would require 45 minutes and our two appointments are only 15 minutes each. Next year, I should schedule us for two extended comprehensive appointments of 60 minutes each to give us a total of two hours to accommodate the 90-minute conjugal-visit portion.
When my wife and I are fashionably dressed in our crinkly, uncomfortable, yet somehow stylish paper gowns, the PA and her assistant come back into the exam room.

The PA goes over our skin with a fine-tooth magnifying glass looking for potential skin cancer spots while the assistant takes notes. To preserve our modesty, the PA shifts the paper gown around as she inspects us here and there and everywhere, although it would be more efficient if the exams were done with us naked without the gowns and underwear, as I have read in so many other posts.
