Patients and doctors: preferences for testicles/sctrotum?

Play patients and/or play doctors: Do you have preferences about whether the testicles are loose in the scrotum, or tighter against the body?

I like a looser scrotum because I like to see the testicles individually and feel them or have them felt as thoroughly as possible.

Being able to see a guy's testicles, or have mine seen, adds to the intimacy of an exam, I think.
