I am in the process of picking out a portable massage table to modify for kinky exam use. I welcome any suggestions as I try to figure out exactly what/how I want to accomplish this task. I would love to have a real table, but I don't have a place I can keep it. My primary interests and plans for this table are OB/GYN exams often combined with Strict Bondage. (We don't want the patient getting away or moving in such a way that they might hurt themselves...) This table needs to remain portable, and preferably can reverted back to stock appearance.

My current plan is to remove the top cover and foam, and add a frame of 1"x4”s or 1/2" plywood flat to reinforce the thin plywood the tables come with. They will have T-nuts in key areas to allow a secondary "accessory" frame to be bolted in from the bottom and removed. Once the foam and cover is replaced, is will appear stock and unmodified except for 3-4 small 3/8 (~1 cm) holes along each edge. The accessory frame will be made of 2x4s mounted flat, and have holes with T-nuts on top every 1-1/2 inches (3.8 cm). I will probably have an additional spacer frame between the top the the accessory frame and the bottom of the table top to make the accessory frame flush with the bottom of the massage table sides. This will also give some room for the bolts in the T-nuts to protrude without running into the bottom of the massage table.
The accessory frame will allow you to mount anything into this frame using a bolt (or more if required) that can be threaded into the T-nuts from below. Or more simply, you could just thread an eye bolt in and use it as a single anchor point. For the horizontal straps, I will use yoga straps and cut them near the buckle and put a grommet near the freshly cut ends of each of the two pieces. This way a bolt can be put through each of the grommets and secure them to opposite sides of the table. If done right, the buckle portion should be just proud of the table top to allow for easy and secure fastening.
Stirrups
I haven't quite figured out the knee crutch stirrups. I think I am going to buy some from eBay. I could make some from PVC as some have described in other posts here, but when considering the time invested and the comfort and adjustability of the final product, I think I am better off buying. The hard part is the clamps for the rod that supports the stirrups. Most use a flat rail that is mounted on the side of exam/operating tables. I can not find the rail by itself for a reasonable price or their dimensions to make my own. I have also seen clamp on candy cane stirrups that use a C-clamp style clamp that could be secured to a little 2x4 stub mounted to the end of the table via the accessory frame. However, I can't find the clamps by themselves.
Arm Boards
I could easily make arm boards that stick out at 90 degrees. I could mount a 2x6 across the bottom of the accessory frame that sticks out on both sides the length of the patient's arms. I would have a padded top that is elevated to match the height of the table top. The problem comes when trying to make the angle adjustable. You could have a centered pivot point with a pin that locks in the angle, but I don't think it would stand up to a patient that likes to struggle.
Head Mobilization Device.
I haven't put a ton of thoughts into this. My first thought is a foam block like you see on spine boards. But mounting those without Velcro attached to the table may be tricky. Also, I enjoy sensory deprivation play, and there wouldn't be room for ear muffs. A couple straps hooked to a collar would not stop the patient from turning their head side to side. Maybe a Cervical collar with a couple D rings attached to the side?
I welcome ideas, suggestions or criticism. Especially if you have any experience to add. However, random advice without experience is still appreciated. I am just trying to get these ideas out of my head and into something that works. If you have anything that I should consider adding, please let me know.
