Upcoming exam

The time has come for another annual exam. I already have butterflies thinking about it. Unlike previous years, there was actually some forms for me to fill out online. I've been having mild prostatitis symptoms again and was on the fence about mentioning it, but wrote it in on my form and committed. My doc is middle aged, tall, attractive, friendly, smart, and dresses formally with a white coat on. He's not the most thorough Ive ever had, perhaps second most, which is quite reasonable given today's standards. He still does a testicular exam unprompted. I do undress and put on a gown. He tells me to undress to my underwear with the gown open to the back. I've had 4 rectal exams from him. 3 for prostatitis, one for bleeding. His go to phrasing is something like, "I'm going to have you leaning over the table. Any blood that you've noticed? Sorry about this, a little cold gel. Keep breathing for me."On the second one for prostatitis, I had said I wanted more testing before continuing antibiotics. He said if things felt boggy he'd do a prostate massage to get a better culture. He had me hold my penis over a sterile specimen cup as he did it. As he was pressing on it, I had to milk my penis, squeezing base to tip until I released a milky drop. One of the most arousing exams ive had. It was difficult to urinate after because my urethra was full of prostatic fluid. The last one was really quick, but still felt nice when he pressed on my prostate. Will update after the exam.

He still does a testicular exam unprompted.

Something my doc's never do. They should, but they don't. Your doc sounds really good, and I'm kind of envious.

@anDRE21 The doctor you described is not the most thorough doctor you've had... That's only the second best...?!? Holy cow, you better hang on to that doctor with all you've got! I can't imagine why on earth you left your #1 doctor.

I got a testicular exam by telling the doctor I was concerned about possible varicose veins in my testicles, which i was. He reassured me that things were normal.

The most thorough doctor I've had was a DO with his own practice. He was my favorite doc when I was living in another state. Tall and athletic guy that used to play ball and was very attractive. He was also very humble and just a genuinely good and caring person. His physicals were very thorough for new patients. He would even try to elicit muscle reflexes on my arms, have me take my shirt off before auscultation and doing the abdominal exam, checked my skin, and etc. With him, if I had any symptoms that could warrant a rectal exam, he was initiating it and apologizing for having to do it. Got my first exam from him after seeing blood at 22. I started to pull my pants up after the genital exam and he stopped me and said, "sorry I'm going to have to do a prostate exam(yes he said prostate). I'm going to have you turn around and bend over the table." He had basketball player hands and took his time with things. His approach was a gentle and gradual insertion with an unhurried exam. The prodding around on my prostate got me fully erect. I had to waddle across the room with my pants around my ankles in front of him, with a hard on, to throw away the tissue after wiping. There's no way he didn't see. He offered to do an anoscopy after that, to which I agreed of course.

With him I got the feeling that he had no qualms about doing a rectal as he would with any indication. I think he may have been gay as he was in his 40s and unmarried. My current doctor will generally do an exam if I steer things in that direction and ask. I'm not sure that he would if I didn't ask. I can tell he doesn't like doing them, but he's still professional about it. With that said, he did offer to do the prostate massage without me mentioning it specifically. My doc that was generally more thorough did not. The urologists also did not.

The exam was better than last years. It was quick, but it seemed thorough by today's standards. It was a crystal clear day and the mountains were all visible driving in. The view from the top floor of the building had a great view of mount rainier. He started with the interview, going over medications, risk factors, and current issues. He'd come in without knocking actually. I brought up the urinary symptoms and asked for a urine culture, sti screening, and a prostate exam. He confirmed he would do the lab stuff and refer me to urology. I requested that he do the prostate exam again and he agreed. Can tell he doesn't like doing them and the previous year he didnt feel like there was any prostatitis present. I knew it would feel boggy this year because my urine flow was obstructed. He said let's give you a gown to change into and provided one before leaving the room. I stripped to my underwear and put one on. He started by shining the otoscope in my eyes and then had me lower my mask to say aaah. Next he palpated my neck and lymph nodes.He placed his stethescope against my bare back and listened to my lungs before checking my moles with the otoscope. After that he pulled the top of my gown down and exposed me upper body. He placed the stethescope against my chest to listen to my heart. After that he pulled the extension out for the table and palpated my abdomen. He lowered the exam table and checked my feet for sensation. After that he had me stand in front of him, place the gown on the table, and lower my briefs. He donned silver nitrile gloves and did an external hernia check without placing his finger into the inguinal ring through the scrotum. Following that he did a quick testicular exam as I looked on, trying not to get erect. He said," let's go ahead and have you lean in on your elbows. Sorry about this. You've been through this exam before(as he says this I hear the cap of the lubricant flip open). No changes in the skin around, you've always had this little bump here? As long as you can remember? Sorry, keep breathing for me. I mean, slightly soft. You doing ok? Painful or not? All right, you're all through. Sorry about that. That was pretty boggy". I felt him circling his finger around my prostate a few times and clenched down on his finger when he hit it. He's in very quick and right on target. Didn't realize how much they could see my penis from the bent over position. Perhaps it was because of how arched my back was. Didn't get hard, but perhaps slightly expanded. Suggested trying low dose cialis to treat it. Have a follow up with urology next week and I'll take that low dose cialis before the visit. Might lead to an embarassing erection.
