After reading through these, I decided to look at my own visit notes. To my surprise, it did note "circumcised". My doctor became part of a larger network a few years ago and seems to have joined their processes. The notes are online and follow the same format yearly, with most of the same feedback. I haven't seen what he's filling out on the computer, but the repetitive nature leads me to believe it's checkboxes or pull downs.
For the physical exam portion the report, it is divided into sections (neck, cardiovascular, chest, genitourinary, etc.). Each section is further broken down (genitourinary: pubic area, penis, testes, etc.) with each having some comments. Looking back a few years, the results are the same, with the "penis" subsection staring with circumcised, followed by additional standard notes.
The only area where I notice changes is occasionally some of the main sections will have a "comments" subsection added in bold. This year, my genitourinary section had the comments: residual foreskin noted easily retracted, glans without rash or lesion, testes descended bilaterally 3+/5, firm. I found this interesting because I've mentioned in previous visits that my penis seems to have shrunk, often appearing uncircumcised. We've discussed it as well, but this was the year it was noted.
