This is not a straightforward yes or no answer, but it's definitely much closer to “No” than to "Yes".
That page, which is the front page, is filtered according to what you have opted in with any classification tags you have selected. As I can't know which tags (if any) you have selected, I can't know how much of the new content you are and are not seeing from that page. Consequently it might be a very narrow view or a fairly broad one, but I'll hazard a guess that it is probably far from complete. Essentially, your mileage may vary.
The front page doesn't show you all of the site's content, historically. For that, you need to go to the top level of the forum and drill down. And when I mention the top level of the forum, that is what you are referring to as “Discussions/Browse”. From there, you can descend to the subordinate classifications underneath each top-level interest, as @sweetkissestoo describes perfectly.
1. Look at the top navigation bar
2. Click on “Discussions” the page will refresh to show you the list of Discussions/Categories that are available.
3. Read down the list of the many Discussions/Categories.
4. Click on any one that interests you to see the slew of sub-categories that are available to read.
Picture the forum and its subcategories as a root and branch structure, and you'll be able to navigate it much more effectively than it seems you have been doing up to now.
P.S. Is the list in What's new/Discussions any different from the list in Discussions/Browse that sometimes presents itself to me (but not initially, I believe)?
Yes, it's potentially very different.
As you do not want to filter anything, I recommend you use “Recent Unseen”, which is one of the buttons you see on the Discussions/Browse" page. That way you will see an unfiltered list of threads with new posts you haven't read. Now, while certain interests such as medical fetish and enemas will be heavily represented in that list, there will probably be a few other things. I'm seeing one now in the Recent Unseen list where members are fantasising about encountering aliens, for example. You may or may not see that from the front page. I know I certainly wouldn't with my own classification tag selections.
This starts to explain why your view of the forum has always seemed to be at variance with mine, so at last we might be getting somewhere.
