Physical exam turning sexual

When you fantasize about being examined, and the exam becomes sexual, does the doctor become a participant in the action or merely an observer?

For me, I much prefer the (female) doctor to examine my penis and testicles thoroughly, but strictly for “medical purposes”. When it comes time for the sperm sample to be generated, I always imagine her sitting at a distance watching.

Honestly I've never had a fantasy like that.

For fantasy: both. For a long time my go to fantasy was that I was a college age woman participating in a study. I was given a very thorough exam and then injected with a drug that lowered/removed inhibitions, placed in a room with other young men and women and then filmed and monitored to see what would happen. Predictably it turned into an orgy.

@Nurse_Phillips that's a study I'd like to be a part of!

I was given a very thorough exam and then injected with a drug that lowered/removed inhibitions, ….

Really? And just what drug might lower your inhibitions, Nurse Phillips? Perhaps a placebo? You could go to town and blame it on that!
