Realistic vs Fantasy Dental exams?

Are you someone who prefers "as realistic" a play dental exam/procedure as possible. As "clinical" as it can be, or do you prefer more playing out a fantasy involving other aspects of kink and BDSM like a dentist/doctor tying you to the chair. If I had to choose one I'd pick realistic, though if ever given the chance I'd love for such an exam to have elements of fantasy at times.
