I would send you a pre-exam questionnaire by email and ask you to complete and return it to me. At your appointment I would bring a hard copy of your form and discuss any relevant points with you. I will ask you to provide a urine sample as soon as you are able, I would test that with a dip-stick. Some women like me to assist or observe them while they provide the sample, others prefer it to be private, that would be your choice. I will provide a single use examination gown for you to wear or you can have it done upskirt UK style. The medical itself would consist of a general medical with the stethoscope etc, a tummy and lower abdomen examination, a breast examination, a gynaecological examination with smear test, and a rectal examination with fingers and a proctoscope. If I do your exam I would have a female nurse assisting me. The nurse would take your medical history and complete the forms. Once completed the nurse would give me the forms to review and take you the the exam room. The nurse will have you take off all your clothes and sit nude on the exam table. Once ready the nurse will come out and tell me you are ready. I will come in and introduce myself and tell you that I noticed you are past due on a couple of vaccinations so we get those up to date first. I need you to get up and lay down on the exam table on your stomach. As you turn over and lay down, I will extend the leg rest. As I do that the nurse comes in with a tray and on it are two syringes with an 18 gauge, 2 inch long needle. The syringes are filled with sterile water so they will sting a bit. I give you the two injections, one in each cheek and you start to get up. I tell you to stay there as I have to take your temperature. So I lubricate your anus and insert the thermometer. I tell you we have to wait 5 minutes so I start asking you questions about your medical forms while we wait the 5 minutes. At the end of the 5 minutes, I take the thermometer out and read it. I tell you that it is 100.5 so you must have some infection, but don't worry we will find out what it is during the exam. I also explain that your complete exam will include a rectal exam so we will have to do three enemas to get you ready. About that time the nurse comes back with the enema bag and nozzle so I lubricate the nozzle and insert it. I open the clamp and tell you if you start cramping to tell me. The enema goes in quickly and when it is fully in, you tell me you need to expel. So I let you get up and go to the bathroom to expel. We then do two more enemas, same procedure and when you finish expelling I tell you to sit on the exam table again. I then do a fairly normal exam, starting with eyes, ears, lymp nodes, listen to heart and lungs. Then I tell you to put your arms behind your head and I do a very complete breast exam of each breast ending with pinching the nipple. Then I tell you to lay down, I again pull out the foot rest, and have you put your arms behind your head again. I repeat the breast exam. Then I tell you I need you to your hands and knees or you turn over and get in that position. You ask me why and I tell you this is a new position for breast exams in addition to the other two. So I repeat the breast exams, taking my time to really feel each breast completely ending with pinching the nipple. I tell you to turn over and lay on your back and I examine your internal organs and listen for bowel sounds. When that is complete I pull out the stirrups and ask you to scoot down and put your feet in the stirrups. I tell you to let your legs fall apart and I do the visual exam of your vulva. I then put on the gloves and tell you I will be examining your vulva and vagina. I feel the vulva, and insert one finger in your vagina. I feel the inside of your vagina and pull my finger out. I then insert a Pederson speculum and spread it. I look with a light and then tell you may feel a pinch as I do the papp test of the cervix. After using both the brush and the wooden tool, I reglove and insert two fingers in your vagina. As I do that I feel for your ovaries and uterus with my other hand. I remove my finger and tell you it is time for the bi-manual exam. I reinsert a finger in your vagina and one in your rectum and feel around to make sure everything is ok. After that I expalin that I need to do a sexual reflex test and for you to relax as I am going to stimulate your clitoris until you have an orgasm. I start a stop watch and as I am rubbing your clitoris, it takes 5 minutes until you explode in an orgasm. I give you a couple of minutes to relax. I then tell you that I noticed a spot that concerns me in your vagina and I need to use a collins speculum so I can see the top and bottom walls of your vagina better. So I insert the collins and spread it. I do a very careful visual inspection and physical inspection of your vagina walls and tell you everything looks ok. I then tell you that I have not found the infection yet, so I am going to insert a catheter and get a sterile urine sample. So I insert it, get the sample, and give it to the nurse to test. At this point I let you get your feet out of the stirrups and tell you I need your on your hands and knees again. When you get in that position I tell you that I need to insert a Pratt rectal spectulum I lubricate you again, insert it, and take a very careful look with my light and tell you everything is OK. About this time the nurse comes in and tells me that the urine shows that you have a UTI and so I ask you to lay down on your stomach again and the nurse has another tray with two more syringes on it. I explain that the antibiotic required for this UTI requires enough volume that it takes two shots, one in each cheek. So I give you two more shots, one in each cheek and tell you that you can sit up. As you sit up I explain that everything is OK except for the UTI, and your will need to come back in 3 days and get another urine test and two more injections. I ask if you have any questions and you say yes, why did I have to be totally nude for the entire exam. I explain that a large university did a very extensive study of women's physical exams, one group had a gown and the other group was totally nude. The group that was totally nude the doctors found more medical problems than the group with the gown. The study concluded that when the woman wears a gown, that the doctor is to concerned with her modesty and covering and uncovering body parts and so he missed symptoms that he should see. So the study concluded that it is best to have the woman nude for the total exam. I ask if there are any more questions and you ask why you can't take pills instead of coming back for more injections. I tell you that they specific type of infections that your have requires injections. So I say I will see you in 3 days and leave and tell you that you can get dressed.
