The genital and rectal exams are the highlights of any adult male physical examination. I find them to be extremely embarrassing and reflect a power shift to the examiner who is getting to see what the man has in his private areas. I am certain that I am not alone in this forum when I admit that I practice masturbation shortly after the completion of every physical exam and that these are the parts of the exam I focus on when I do it. I concentrate on the anxiety leading into these parts of the exam, the process the examiner follows, both verbal and nonverbal components, my emotions as the inevitability that these parts of the exam are going to happen, how I feel when the doctor sees my manhood, and the way the doctor inspects and explores my private parts.
As for the genital exam, I have had this performed lying supine with the examiner lowering my underpants to expose me and standing in front of a standing or seated examiner. While it is embarrassing to have someone drop your undershorts and check out your male parts while you are lying on your back,personally I find it a much more humiliating to stand in front of a seated, fully clothed examiner and be instructed to lower your shorts to present your man parts for inspection. It creates a sense that the patient is presenting his private male parts for inspection and approval. I always wonder if the doctor thinks my size and appearance are acceptable for a real man. I also hope that the doctor was looking forward to the moment he gets to see what I have got.
As far as the position for the rectal exam, I have experienced them in the Sims position as well as bent over the exam table. One that was done in Sims was after the abdominal and prior to the genital exam. I found it more awkward to get into the Sims position and more awkward for the examiner to expose and enter the anus since he has to lift the cheek to expose and enter the anus. While I like the awkwardness of the positioning for doing the exam in the Sims position, I think overall it is more embarrassing to have the exam done bent over the table. It gives the doctor a clear view of the anus and the penis and scrotum hanging down between the patient’s legs. I have not had the experience of having an examiner instruct me to spread my cheeks for him which I think would create the ideal experience with a rectal exam.
To summarize my thoughts, I will express my preference for the genital exam being done standing in front of a seated examiner and the rectal done bent over the table with the patient spreading his own cheeks.
