Emma gladly sat up, letting her legs dangle off the exam table between the stirrups. Alex rubbed her back as the doctor spoke.

“So it looks like your IUD got pulled down from your uterus to your cervix. The strings attached are there for removal, but in very uncommon cases they can be pulled during intercourse, masturbation, or even while using tampons. The IUD then irritated your cervix, making it sensitive to touch.”

“That’s great!” cheered Alex. “So you can just remove it?”

“Yes, it does need to be removed. Especially since it is nearly expired anyway, if it was placed 7 years ago. However this can also be a painful process, especially if your cervix is already irritated from the IUD scratching from the inside. But we can give you some pain medication to help. What do you think Emma?”

Emma appreciated the doctor, during the insertion they had told her it would be painless only to leave her bleeding and cramping for days. But she still didn’t want to go through all that again. “Is there any alternatives?”

“Maybe. I could refer you to a surgeon who may be able to remove it under general anaesthetic. You would likely have to wait a few months, potentially with the pain getting more frequent and more severe during that time.”

Emma nodded as the doctor spoke. “Ok doctor, I want it removed today, with whatever pain relief I can have.”

“Good, so with these painkillers we will need to wait a bit for them to take effect, but they also reduce muscle spasms which should help you relax for the procedure. I’ll write you a prescription and have Noah get you ready. I’ll be back shortly.”

As the door closed behind the doctor, Alex gave Emma a reassuring smile and back rub. “See, you’re in such good hands! It’s all figured out and will get fixed today!”

Noah came back in carrying a small tray before Emma could reply. “Hi again Emma! I hear we’re having a little procedure. The doctor said you’ll need some pain management for this, and the best thing is a suppository, since that is right by the affected area. They also take effect much faster so you won’t have to wait as long. When was your last bowel movement?”

Emmas face dropped. “Oh, I don’t know. This morning?”

Noah remained as cheerful as ever. “That’s great, you should be just fine to take these rectally then.” He placed the tray with a few small packages onto a portable table. Alex recognised the same lubrication that had been used with the thermometer, but nothing else.

“Emma we’ll have you lay down on your left side, just here.” For the first time, Emma didn’t hesitate or question, she just lay down and let Noah position her legs, bending her knee towards her chest and uncovering her rear from under the gown. The doctor came back in as she was being positioned.

“That’s perfect Noah, thank you. Just hang on in here until we’re done. Emma do you have any questions about the medication?”

“No, I don’t think so…”

“Ok let’s get started then. I’m going to insert a finger into your rectum first, checking for anything that might be contributing to pain or discomfort. This might feel a little cold, take a deep breath in for me…” The doctors cold index finer gently pressed against Emmas anus while she inhaled. “Try and relax, now exhale and bear down.”

With that, her index finger disappeared into Emma’s rectum, her anus spasming briefly at the intrusion. The doctor stood up from the stool to rotate her finger all the way around, back, and around again. She was very thorough. “I’m going to remove my finger now Emma, bear down again.”

Emma did as she was told, with only minor squirming during the exam. She had never had so many things up her butt, but was too tired to do anything but submit.

“You’ll get two suppositories Emma, just hang on there.” The first was poised and ready. “Deep breath in… Relax… Exhale… Good.” The bullet shaped medicine slid easily inside since Emma was so well lubricated, followed by the tip of the doctors finger to make sure it wouldn’t come back out.

“One more Emma, breathe in… and out…” This time the doctors whole finger followed the suppository, pushing it deep and holding it in. Emma wiggled and clenched, leading the doctor to place her free hand on Emma’s hip to encourage her to be still.

“Now Emma” the doctor started, finger still fully inserted, “these will slowly dissolve over the next 15 minutes, so we can’t let you sit up until then. I’ll remove my finger now, but don’t bear down as we don’t want any medication coming out. Ready?” The doctor removed her finger and gave Emma’s hip a small pat to comfort her. “Roll onto your front, Noah will stay with you while those take effect. Let me know if there are any problems or questions.”

With that the doctor binned the used gloved and left the exam room again. Noah immediately helped Emma by positioning her shoulders and legs to lie face down again.
