I guide Mookie to the exam room door and bring in Kevin for his exam. Kevin’s exam starts the same as Mookie’s. Medical history interview, only notable concern is a broken arm in high school, fully healed with no restrictions. Quick check of Kevin’s vitals just like Mookie, 123/85 blood pressure, 76 pulse rate, 18 respiratory rate, 98.7 temp and 96% Spo2. Kevin’s chest is somewhat hairy so I had to shave enough for the ekg stickers on his chest. Again I tell him to lay back and relax so I can take the reading, which was a Normal sinus rhythm as expected.

I begin my physical exam of Kevin as I did with Mookie, examining his light blue eyes with the otoscope as well as his ears and nose; then have him follow my finger with his eyes to check movement and reflexes. Open wide and say ahh as I used a tongue depressor to check his throat. I palpated his neck and asked him to swallow, felt his gums and teeth with my finger.

I listen to Kevin’s chest and back, lightly grazing his nipples and having Kevin take several deep breaths. When I grazed Kevin’s nipples and they turned erect and Kevin was ticklish and gave a slight giggle as I palpated his chest, abdomen and armpits too. When I reached inside his waistband I noticed he had become plump, and I had warned him that my hands were slightly cold but had warmed up a bit. I had him reach his arms above his head and palpated Kevin’s armpits and checked for rash/irritation.

Next was the reflex and feet exam. Using a hammer I checked his elbows, wrists, knees and ankles. I also scratched the soles of his feet which made his toes curl and he giggled again.

Last but not least was again the fun parts for me and not so fun for Kevin. I asked Kevin to stand and remove his shorts so I could examine his privates and rectum. Kevin did and whoa. I kept it professional but I’m not sure how. He had a shower that was about 6.5-7 and growing. Nice bush of red hair and a decent sized set of testicles. I begin my exam by having Kevin bend over and try to touch his toes to check his back. I palpate each vertebra from his neck to his coccyx like Mookie, ask if he feels any pain and use the instrument to check the angle of his spine. Kevin stands back up and I begin examining his uncircumcised penis and testicles, raking my fingers through his pubes and giving his penis a few tugs. Kevin isn’t as nervous as Mookie since he had thorough sports physicals in college that always included a hernia check. I tell Kevin to turn his head to the left and cough three times on each side, and examine each of his testicles. I ask about sexual health and partners, and Kevin said he’s been monogamous with his girlfriend for five years and they usually use condoms.

Next comes my favorite part, the rectal exam.

I told Kevin it would be quick, and with as little discomfort as possible. Just like with Mookie, Kevin then bent over the exam table, standing on the platform, and spread his cheeks open. I started with an external exam and palpated his sphincter lightly. I then applied a generous squirt of lubricant to my index and middle fingers and told Kevin to count to 3. On three I inserted my pointer finger and then briefly pulled out and reinserted with the middle finger, palpating for any hemorrhoids or other abnormalities. I reach Kevin’s prostate and he moans pretty loud. I give it a really thorough check and he moans again and grunts. He whimpers that he is going to ejaculate. I get a sample cup for him and he provides a large semen sample, as I hear it hitting the cup while I finish the exam. Kevin also passed the exam, and I let him pull the shorts back on and go get dressed. Once dressed, I told Kevin he could go meet with his liaison to finish up and I would see him before he left for basic. Kevin also seemed happy that I would see him before he left.
