Hanna's Unexpected Uterus Removal

Initial Consultation

Hanna and her friend Ramesh arrived at this Hospital, their expressions a mix of concern and determination. After completing the necessary paperwork, they made their way to the Gynecology Department, where they were led to Dr. Kalaivani Gunasekaran's office.

"Good afternoon, Dr. Kalaivani," Hanna greeted the doctor with a nervous voice, her eyes darting between the doctor and Ramesh, who stood supportively behind her.

Dr. Kalaivani, clad in a graceful saree, gestured for Hanna and Ramesh to come in and take a seat opposite her. "Good afternoon, Hanna. Please, have a seat. How can I assist you today?"

Hanna took a deep breath, her hands trembling slightly as she spoke. "I've been experiencing pain in my lower abdomen, and I've noticed some bleeding through my vagina at times. My family has a history of cervical cancer, and I'm really worried."

Dr. Kalaivani listened attentively, her expression one of empathy and concern. "I understand your concerns, Hanna. Let's go through your health history in detail to get a better understanding of your situation."

As Hanna shared her medical history, Dr. Kalaivani took notes, asking questions to gather more information. "Have you experienced any other symptoms, such as unusual discharge, weight loss, or fatigue?"

Hanna shook her head. "No, just the pain and occasional bleeding."

"Have you had any previous surgeries or medical conditions?" Dr. Kalaivani inquired.

"No, this is the first time I've had such issues," Hanna replied.

Dr. Kalaivani nodded, taking a moment to consider the information. "Given your symptoms and family history, it's important that we conduct a thorough examination. One of the procedures we may need to perform is a dilation and curettage (D&C) to obtain a tissue sample from your uterus for further analysis."

Dr. Kalaivani leaned forward, her tone gentle yet firm. "If the biopsy reveals any malignancy, we would need to consider further treatment options. In some cases, this may involve the surgical removal of the uterus and ovaries to prevent the spread of cancer. However, let's take this one step at a time and focus on the initial examination and D&C first."

Hanna took a deep breath, trying to steady herself. "Doctor, I am sure, the results will probably indicate the same. I had this test done a month ago while I was in US and it did indicate malignancy. I dont have the test reports but I wish you will agree to operate on me and save me from this"

"We'll take good care of you, Hanna," Dr. Kalaivani assured her. "Let's schedule the necessary tests and proceed from there..

Kalaivani began the examination by asking Hanna a series of detailed questions about her symptoms, medical history, and overall health. With each response, Dr. Kalaivani took careful notes, ensuring she had a comprehensive understanding of Hanna's condition.

"Alright, Hanna," Dr. Kalaivani said gently, "I'm going to conduct a physical examination now. Please take off your pants and underwear and lie on the exam table, and we'll get started."

Hanna, bit startled , feeling a mixture of anxiety and helplessness, and to an extent excitement undressed and lay down on the examination table, her heart pounding in her chest. Dr. Kalaivani approached with a strict demeanor as she proceeded to examine Hanna.

"First, I'll perform a general physical examination to check your overall health," Dr. Kalaivani said, using her stethoscope to listen to Hanna's heartbeat and lungs. "Everything sounds normal so far."

Next, Dr. Kalaivani conducted a pelvic examination, carefully inspecting the lower abdomen and reproductive organs. "I'm checking for any abnormalities or signs of infection," she inserted her gloved fingers of right hand in Hanna’s vagina, while palpating Hanna;s lower abdomen with her left hand. "Please let me know if you experience any discomfort."

Hanna felt a slight pressure but remained composed, awed by Dr. Kalaivani's professionalism and approach and the coldness in her style. After completing the examination, Dr. Kalaivani helped Hanna sit up and get comfortable.

"Based on the examination and your symptoms, I recommend we proceed with a few diagnostic tests, including a dilation and curettage (D&C) to obtain a tissue sample," Dr. Kalaivani said, meeting Hanna's eyes with a reassuring gaze.

Hanna underwent the dilation and curettage (D&C) procedure. The hospital staff were supportive and efficient, ensuring Hanna was as comfortable as possible throughout the process. She was given a bed in a quiet room to recover, with Ramesh by her side. Hanna & Ramesh were asked to come back the next day for the results.

After some rest , Nervous but determined, she and Ramesh made their way back to the Gynecology Department.

Dr. Kalaivani welcomed them into her office with a warm smile. "Hanna, Ramesh, please come in and have a seat."

Hanna's heart pounded in her chest as she sat down. "Doctor, can you proceed with the treatment without waiting for D & C report" she asked, her voice trembling.

Dr. Kalaivani glanced at the file in her hands and then met Hanna's gaze with a serious expression. "Hanna, Are you sure? This means we need to discuss your treatment options moving forward and they are not very pleasant."

Hanna felt a wave of fear wash over her, but she steadied herself, knowing she needed to face this head-on. "What are my options?"

Dr. Kalaivani explained, "Given the possibility of malignancy, one of the recommended treatments is a surgical procedure to remove the uterus and ovaries. This is to prevent the cancer from spreading further. Of course, this decision is ultimately yours, and we'll support you in whatever choice you make."

Ramesh squeezed Hanna's hand, offering silent support. Hanna took a deep breath, considering the weight of the decision before her. "I understand, doctor. I think I want to get done with it."

"Of course, Hanna," Dr. Kalaivani said gently. "Take some time to think over, You need not rush into this decision like this . We'll schedule a follow-up appointment to discuss your decision and plan the next steps."

Hanna nodded, feeling both overwhelmed and relieved to have a clear path forward. Hanna was having her dream of being wheeled into surgery come true in front of her eyes, She was both excited and nervous. She felt all this was surreal and she was not expecting this to happen this way. She knew that the journey ahead would be challenging and could turn out in ways she was not expecting, but she was determined to have her trill and make the best decision for her mental well being.

As Hanna and Ramesh sat in the doctor's office, Ramesh couldn't help but ask, "Dr. Kalaivani, do you think the cancer could have spread to more areas?"

Dr. Kalaivani, with a thoughtful expression, replied, "Based on the density of cells in the sample we obtained, it is possible that the cancer has spread, but not too far. But for that we have to wait for pathology reports and if pathology reports turn out to be as what Hanna is expecting them to be, I am afraid we may have to go for more extensive procedure. In that case, to ensure we address all potential areas, I strongly recommend that we perform an omentectomy on Henna along with the hysterectomy and oophorectomy.It can be bit stressful for both of you since the surgery will be longer and more painful with a longer incision "

Hearing the word incision, Hanna was actually excited, though she tried to maintain a puzzled look. Dr. Kalaivani continued, "A hysterectomy is the surgical removal of the uterus, while an oophorectomy involves removing the ovaries. An omentectomy is the removal of the omentum, a layer of fatty tissue that covers and supports the intestines and organs in the lower abdomen. Removing these tissues will help us ensure that we remove all the affected areas and reduce the risk of the cancer spreading further."

Ramesh asked, "Can these procedures be done using keyhole surgery?"

Dr. Kalaivani shook her head gently. "Unfortunately, given the complexity and extent of the surgery, a laparoscopic or keyhole approach is not suitable. We will need to perform an open abdomen approach, which allows us to have a better view and access to the affected areas."

"Additionally," she added, "I will be collaborating with Dr. Santosh Kaur from the Oncology Surgery department. Together, we will perform the surgery to ensure comprehensive care and expertise."

Hanna felt a mix of anxiety and relief. With Ramesh's support and Dr. Kalaivani's guidance, she felt ready to face the challenges ahead. "Thank you, doctor. I'll do as you recommend."

Dr. Kalaivani smiled warmly. "We'll take good care of you, Hanna. Let's get you admitted and begin the preparations."
